This form is used by field personnel to register a complaint or describe a problem with Lancaster County Communications. After receiving the completed form, the supervisory staff conducts an investigation into the incident.

Location Safety Alert - Critical Property Information Form

Use this form to flag an address with responder safety information or to update the critical information about the address.
